Blood Test Instructions for Potential Donors

Your amazing breast milk will be prescribed to premature and critically ill babies. To provide the purest milk possible, we require all donors to complete a blood test to screen for harmful viruses.

Thank you again for your donation. Please read these instructions carefully and call us at 617-527-6263 ext. 3 with any questions.

Enclosed in this package you will find:

  • Rhode Island Blood Center Service Requisition form – please date on the top right hand corner the date and time you have your blood drawn.
  • Packing Materials for mailing the tubes – please put the gel pack in your freezer.
  • Three pre-labeled special tubes for blood collection located inside the safTPak box enclosed within the multiple layered bags.

Please take the enclosed tubes and requisition to your PCP/OBGYN/or PEDI or local lab to have your blood drawn.  Blood should be drawn Monday thru Thursday ONLY.

If you are having a problem finding someone willing to volunteer the time and syringe required to draw your blood, please call us at the Milk Bank at 617-527-6263 ext. 3.

We have contracted with the Rhode Island Blood Center (RIBC) for your blood work at no cost to you.

**Please note that RIBC must receive the package within 72 hours, so it is important that FedEx receives your package on the day that your blood is drawn**

Packaging Instructions:

  1. Once blood work is drawn, place the tubes in the bubble envelope.
  2. Place the bubble wrap envelope into the SafTPak STP-710 white envelope and seal it.
  3. Place the frozen gel pack into the SafTPak biohazard bag.
  4. Insert the white envelope into the biohazard bag and seal.
  5. Place sealed biohazard bag into the SafTPak STP-210 box.
  6. Place the RIBC requisition into the manila envelope.  MAKE SURE THE PERSON DRAWING THE BLOOD HAS DATED AND TIMED THE REQUISION FORM. RIBC WILL NOT RUN THE BLOOD SAMPLE WITHOUT THIS INFORMATION.
  7. Put the box and the envelope into the pre-paid and pre-labeled  FedEx UN 3373 overpack.
  8. Call PeriShip at 866-928-6428 to arrange for pickup.  Please do NOT put the specimen in a FedEx drop box.
